Request for Return Goods Authorization

We will provide a full certified inspection of all products returned for repair to our facility in Santa Clara, CA. Before shipping a product to our facility, the you must fill out the below Returned Goods Authorization (RGA) form. In turn, a RGA number will be issued and sent to the e-mail address that you provide below with further instructions. *For returns outside of the United States, please contact our sales offices or an authorized distributor.

NOTE: When a rotor is to be returned to FIBERLite™ Centrifuge for any reason, please include the complete rotor assembly (lids, knobs, O-ring, etc.), a history of usage, reason for return, and please make sure that the rotor is safe to handle (non-radioactive and pathogenically clean). Thank you.
